NEW STUDENTS:

 

If you are new to Wagging Willow, please complete the NEW STUDENT application below. The application will provide us with the information we need to get to know you and your canine companion.   Don't forget to upload your pup's vet records and photo!  

 

In addition, please complete the REGISTER FOR CLASS form to register for the specific day and time that your class is available.  

 

RETURNING STUDENTS:

 

 If you have already completed the New Student Application, you need only to complete the REGISTER FOR CLASS form to register for the specific day and time that your class is available.   Please be sure we have your most current vet records on file!

 

PAYMENT:  

 

At this time Wagging Willow accepts cash, check, and cash tranfers via Zelle. Full payment is due by the end of the first class.

 

Checks should be made payable to Wagging Willow.
